A lot of patients put off gum work because of time, pain, or money — then they ask for “just a regular cleaning” when the gums are already inflamed. Scaling and root planing is not that cleaning. It is the periodontal therapy used when tartar and bacteria have gone below the gumline, and a polish on top will not reach them. This page is the procedure. Disease staging lives on our gum disease treatment page; the Cypress write-up is the gum disease treatment in Cypress article.

What scaling and root planing actually is

Scaling removes plaque and tartar from the tooth and from under the gum. Root planing smooths the root surface so the gum can reattach instead of staying as a pocket. It is done in sections, usually by quadrant, with local anesthetic. It is not a “deep spa cleaning,” and it is not the same appointment as a routine prophy.

If a hygienist or dentist has already told you that you have pocketing, bleeding that does not stop, or tartar you cannot see, that is the conversation this procedure answers. If you have not been evaluated, start with the gum-disease visit — we will not sell you scaling and root planing over the phone.

Why a regular cleaning is the wrong treatment on active gum disease

A prophylaxis (the routine cleaning) is built for gums that are healthy enough that deposits sit above the gumline. Doing a prophy on active periodontitis leaves the infection in the pocket. That is why patients can feel like they “just had a cleaning” and still bleed the next week. The clinical decision is staging, then the right instrument set — not a more expensive version of the same appointment.

Dr. Huynh and Dr. Choe keep this in-house. You are not referred out for the therapy itself. If a true periodontal specialist is the right next step after staging, we will say so instead of stretching a general-practice appointment past what it can do.

What the visit looks like

  1. Staging first. Pocket measurements, bleeding points, and X-rays so we are treating the mouth in front of us, not a search query.
  2. Anesthetic and sectioning. Local anesthetic is normal. Nitrous is available if you want it. IV sedation is a separate, quoted appointment when anxiety or appointment length is the blocker — flat $500 / $800 / $1,000 by time, same for every patient. Details live on IV sedation dentistry.
  3. Scaling and root planing. Usually one or two visits depending on how many quadrants need work.
  4. Re-evaluation. Pockets are remeasured. Maintenance is not optional if you want the result to hold.

Time, pain, money — the honest version

Time. Same-day completion is the default when it is clinically reasonable. Splitting quadrants is about anesthetic and tissue, not padding the calendar.

Pain. The procedure is done numb. Soreness after is real; we will tell you what to expect before you leave. Infected tissue is also why local anesthetic can feel incomplete on an emergency tooth — that is a different problem, and sedation is the tool when it is.

Money. We quote after staging, in writing. Insurance often codes this differently from a regular cleaning; we submit, we do not promise the carrier’s decision. Cherry and CareCredit are the financing options if you need monthly payments.

Scaling and root planing questions

Is scaling and root planing the same as a regular cleaning?

No. A regular cleaning (prophy) is for healthy gums. Scaling and root planing treats deposits and bacteria below the gumline when pockets are already present.

Does gum disease treatment in Cypress always mean scaling and root planing?

No. Staging decides. Early gingivitis can reverse with a different plan. Periodontitis is when this procedure is on the table. See gum disease treatment for how we stage it.

Will I be asleep for scaling and root planing?

Most patients do it with local anesthetic, with or without nitrous. IV sedation is available when you want to be deeply relaxed — it is moderate sedation, not general anesthesia, and it is quoted at $500 / $800 / $1,000. Fasting and a driver are required; we do not do same-day IV.

How many visits does it take?

Often one or two, by quadrant, after the evaluation. We would rather finish what we start than split it without a clinical reason.
